You have to fill out a brief questionnaire before getting a massage. It asks for your name, address, phone number, email, etc. (I just filled in a name and an address.) There's also a list of check-the-box medical questions followed by a long paragraph about liability, therapeutic massage only. A signature is required at the bottom of the form.
